Vial of Life Program
The Vial of Life Program is a joint public service venture sponsored by the Sharon Fire and Health Departments. The Vial of Life is a small plastic container that provides a place for you to store important medical information that emergency medical personnel need in a time of crisis. By completing the Vial of Life form now, you can provide emergency personnel with vital information which may save your life during the first minutes of critical illness or accident in your home.
The Vial of Life program is available free as a public service for residents of Sharon. All information provided will remain completely confidential and will only be used by emergency medical personnel in potentially life-threatening situations.
How to use the Vial of Life:
Obtain a Vial of Life form from the Sharon Board of Health office at Town Hall or from the Fire Department.
Complete the form as fully as possible ---- and return it to the Health Office.
You will be notified shortly to return to pick up a computerized printout of the medical information you provided. This will be small enough to fit inside your Vial of Life.
Place the Vial of Life (with the information inside the vial) into your freezer.
Place the Vial of Life magnet (which you will also receive) on the front of your refrigerator door ------ emergency medical personnel in the community have been notified to look for your Vial of Life magnet.
You will also receive a small wallet-sized card providing all your information — carry this with you in case of emergency.
The Vial of Life program has already been utilized in Sharon several times to help save seconds and thus lives. Take advantage of this free public service as soon as possible.
